This form is used to cancel a Pass/Fail request and revert back to the a graded status for a given course. The course will then be recorded with a grade on your grade report and transcript.
Deadlines and Submissions
Students who elect to complete a course on a Pass/Fail basis may request to revert to taking the course on a graded basis up until the last date of classes corresponding to the term of the course. Check the before you submit your request.
