The claim submission process entails, at minimum:

• preparing a correctly
• receiving a doctor's note stating your diagnosis
• getting an itemized invoice from the doctor鈥檚 office (containing OHIP billing codes, which look like this example: A007 or A007A)
• being issued a payment receipt (regardless of the method of payment)

To go over the claim submission process, please prepare the documents above, complete , and email uhip@yorku.ca to send us your documents and ask us if anything else is required. Situations where additional documents are commonly required are:

• you saw a medical specialist, went to the hospital or got laboratory or diagnostic testing
• you will receive follow-up medical services due to an upcoming medical treatment or surgery

When you come to the Claim Submission Blitz, please bring your laptop. The UHIP Office will answer any follow-up questions you have, call medical service providers and Cowan with you to obtain missing claim-related information, and then show you how to submit your Cowan claim step-by-step. This way, you can leave the UHIP Office with peace of mind after submitting your claim.
